What is an SGMS EMR?
An EMR is a unique medical jump/flash drive which you can wear on your neck, key ring or wrist which contains your complete medical history for use in an emergency by first responders or medical personnel How do my medical records and information get transferred onto my SGMS EMR? The information can be entered by you the patient by simply plugging the EMR into your computer and then answering the questions as they appear or, we at SGMS can do it for you. You have to fill in the medical records release form and give it to your doctor, dentist or administrator of the hospital requesting that they forward ALL your records to our Medical Director, Douglas Slavin MD @ 21301 NE 20th Ave., Miami, Fl. 33179.. upon completion, we will send your EMR to you and the records back to whoever sent them to us along with a copy of our program for their use in their computer. What if my doctor doesn't use your program? No need to worry, all you have to do is tell your doctor to plug the Medi-band or medi-chip into his computer and all the information opens up. He can add your medical history or any updates right to the program. If he or she would like the program, please provide us with his/her name and address and we will send it to them FREE OF CHARGE for use with your records and his other patients. Is my information secure and who has access to my information? Your information is very secure because of a special encryption process in each and every Medi-Drive, and in keeping with the HIPPA rules and regulations, only you have the right to determine who sees your records. Each Medi-Drive has two portions, one for the patient, the other for the physician or medical facility, your personal information is kept away from the physicians portion and they only see the medical information unless you permit them to see everything. How do I know that the information on my SGMS EMR (medical flash drive) is correct? If you enter the information, we request that a hold harmless agreement be signed stating that you entered the information and that you hold us , SGMS Corp, harmless if anything is wrong. If we enter the information, we have the institution or doctor/dentist aver that this is correct from their files to the best of their knowledge. It is in this manner that we make sure it is a true and accurate copy of your records. We also ask you to check over everything prior to wearing or carrying the medi-drive. Not really, it was designed for only one person’s information, should you need a 2nd or 3rd drive for family members, SGMS Corp. will be glad to provide you with a 15% discounts for the additional drives. What else can I store on my SGMS EMR (medical flash drive)? Almost anything can be stored on the Medi-Drive, from passport information to drivers licenses, credit card accounts, to photos of your home and possessions, Stock certificates, bonds, collectible serial numbers, even bank accounts, tax returns, the list is endless… How do I update information on my SGMS EMR (medical flash drive)? What if some of my information gets erased? The information can simply be re-entered by either you, your physician or us. If my SGMS EMR (medical flash drive) gets lost or stolen, how do I keep people from seeing my private personal information? The information can be encrypted so no one but you can access it. So if it is lost or stolen, you don’t have anything to worry about. However if you did not encrypt the information, you can password protect portions of the program if you wish, such as your social security #, address, phone number etc: How long will the information remain on my SGMS EMR (medical flash drive)? The normal time is approximately 10 years, however it is advisable to back up your information on a regular basis. You can download the information to your hard drive and then copy it to a disc. In this fashion, you will always have a copy of all your vital information. Can I protect my information with a password or pin? You can select a PIN number, password, or use the encryption program in every Medi-Drive How will emergency medical personnel and first responders know that I have an EMR (medical flash drive)? If you viewed our intro movie you will notice that over 90% of first responders and medical personnel look on the wrist, neck or key ring for some form of medical information or alert tags. Also our Medi-Drives have the universal “STAR OF LIFE” embossed on every medi-drive which is recognized world wide. If my SGMS EMR (medical flash drive) is password protected, how will emergency personal and first responders access my information if I am unable to communicate or unconscious? There are two partitions to each drive, one for the patient and the other for medical personnel. The medical personnel portion only holds the medical information and the persons name and phone number and emergency contact, no other information will be seen unless permitted by the patient. Do you offer a card that I can print out and carry in my wallet to alert others to check my SGMS EMR (medical flash drive) for vital medication information? Yes we do and it in turn mentions that you are carrying an SGMS EMR and to look for it on your person. Will my SGMS EMR (medical flash drive) work in any computer, Mac or PC? Absolutely, as a matter of fact, President Bush said that if he could he would make it a law. By the year 2015 he mandates that every American must carry an EMR of some type with them. Also to bolster his Border Act, the person should have at least 3-5 forms of electronic un-alterable I.D. in it as well. How can emergency medical personnel and first responders be assured that the data contained on the drive is accurate and valid? Once again, if you enter the information, it is only as accurate as what you put in, if we or a physician enters the information we ask you to verify the validity prior to inserting it into the EMR. What is the legal liability for SGMS and emergency medical personnel if the data is outdated or inaccurate or if my SGMS EMR (medical flash drive) malfunctions and can't be read? As for the data being outdated, medical history is just that a history of a persons “medical” treatment, and as such can not be out dated. If the information is inaccurate after you, the patient, aver that it is correct, the liability is then resting on your shoulders. As for the legal liability of the drive mal-functioning, each medi-drive comes with a two (2) YEAR GUARANTEE and because you sign a hold harmless agreement releasing us from any liability at the onset, SGMS Corp has no liability. Where is the best place to keep a back-up copy of my data in case my SGMS EMR (medical flash drive) malfunctions? It is recommended that you print out a copy and keep it in a safe place, a bank safety deposit box is a good place, or enter and copy the data to your computer. Finally you could purchase a second drive and fill it in and then place that drive in a secure place. How can I request more information?
